Our Single Point of Contact team based in Fort Southwick, Fareham, are an essential first point of contact for those referring into our community services across Fareham, Gosport and South East Hampshire. It is thanks to this team that our patients, GPs and carers can access the professional healthcare support they can expect.
If you come to work with us, you will work with the team to cover the Single Point of Contact referral service, taking referrals from clinicians and patients requesting community services intervention and support to prevent hospital admission and for identified single service referrals as agreed.
You will be required to complete the necessary referral form accurately with appropriate checks to present to health professionals with full, up to date information prior to patient assessment. This role involves providing complex information and using knowledge gained to liaise with a variety of health professionals including GPs, to identify and co-ordinate the appropriate action plan for the patients.
You will negotiate with multiple therapies/agencies and ensure an appropriate speed of response for the completion of multi-disciplinary assessment of patient needs.
At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.
