We are looking for an experienced and compassionate Registered Nurse to join our friendly Community Nursing Team, based in Fareham. This role is a fixed term contract of 12 months maternity cover.
In this role you will work alongside patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage and commitment. Care is based on comprehensive assessment, ensures continuity and is patient‑centred, culturally sensitive and evidence‑based and which promotes independence.
As a Community Nurse you will be responsible for completing patient visits each day, influencing clinical decisions and monitoring quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to your own field of practice, underpinned by theory and expertise.
