Overview

Working within professional guidelines and accountability, the post-holder will manage a defined community caseload for a group of GP Practices as part of the Integrated Care Team, using Nursing, Physiotherapy or Occupational Therapy interventions.

Responsibilities

  • Coordinate and delegate work, ensuring referrals are managed according to clinical need.
  • Complete holistic assessments and maintain care plans in line with Trust and professional record-keeping standards.
  • Ensure clinical care consistently meets high standards of patient safety and quality outcomes.
  • Operate with a high degree of autonomy and professional accountability.
  • Aim to prevent unnecessary hospital admissions and support timely discharge into the community.
  • Contribute to re-enablement, rehabilitation, health protection and health promotion programmes to improve outcomes and reduce inequalities.
  • Where applicable, flexible working options can be discussed during the interview process.

  • Full driving licence and access to a vehicle for work purposes (requirement of the role).
