We are recruiting a Band 7 Team Lead to lead a newly established Community Rehabilitation Team (CRT) within the Mid and North Hampshire Division. This is an exciting opportunity to shape and lead a community-based rehabilitation service, supporting adults with complex mental health needs to achieve recovery and independence in the least restrictive environment.
You will provide clinical and operational leadership, ensuring the delivery of high-quality, recovery-focused care across multiple localities, while driving service development and integration with system partners. The service focuses on functional recovery and independence, delivering flexible, community-based interventions across multiple localities.
As Team Leader, key elements of the role include:
- Providing leadership to a multidisciplinary team delivering rehabilitation interventions
- Ensuring consistent delivery of care aligned to evidence-based practice and NICE guidance
- Overseeing service performance, including waiting times, caseloads and outcomes
- Supporting smooth transitions between inpatient and community services
- Leading collaboration across system partners to improve pathways and integration
- Driving quality improvement, audit and service evaluation activity
- Maintaining oversight of safeguarding and clinical risk within the service
