Overview
We are seeking applications from Nurses or other registered Healthcare Professionals to work with us in North Hampshire supporting patients with Multiple Sclerosis (MS). Are you enthusiastic, with excellent interpersonal skills and a genuine commitment to improving the lives of people living with MS?
A keen interest in MS supported by experience of working with patients with MS/neurological conditions, as well as an MS qualification or a willingness to undertake this in the future is essential.
Responsibilities
- Provide specialist nursing care for a defined patient/group as a core member of the multidisciplinary team.
- Deliver specialist case management and educational support for patients with Multiple Sclerosis (MS) under supervision.
- Coordinate care with partner agencies and key stakeholders to promote well-being and education for patients and healthcare professionals.
- Help prevent hospital admission by maintaining safe and effective care in the community setting.
- Report to and be accountable to the Senior Multple Sclerosis Specialist Nurse Lead for North Hampshire.
Qualifications and Experience
- Registered Nurse or other registered Healthcare Professional with interest in MS and neurological conditions.
- Experience in working with patients with MS or neurological conditions.
- MS qualification or willingness to undertake this in the future.
- We will consider applications for a Band 5-6 development role with opportunities to develop specialist knowledge in MS care and work alongside experienced clinicians.
