Join our Admission Avoidance Team as an Admission Avoidance Clinician and play a vital role in supporting patients to receive the right care, in the right place, at the right time. Working within the Transfer of Care Hub, you will assess patients within urgent and emergency care settings, identify alternatives to hospital admission where appropriate and facilitate safe, timely discharge into community services.
You will work closely with multidisciplinary colleagues across acute, community, primary care and social care services, helping to coordinate care, reduce avoidable admissions and support the delivery of Discharge to Assess and Home First pathways.
