Your role:

This is a rewarding position supporting residents to regain independence and return to their own homes.

As a Reablement Support Worker, you will provide multidisciplinary programs of Reablement across the borough, working as part of Urgent Care alongside therapists, social care workers, housing, and health professionals.

The Reablement service operates 7 days a week, 365 days a year, including bank holidays, from 07:00 to 22:00. The role may require working beyond normal hours based on service needs.

You will work every other weekend, from 08:00 to 22:00.

About you:

You should be creative, self-motivated, and customer-focused. Good interpersonal and communication skills are essential, along with a commitment to promoting independence.

You will support individuals in their homes, using an asset-based approach to help them remain independent and connect with their communities to improve well-being.

A valid driving license and the ability to travel as required are necessary.

This role requires an enhanced DBS check.
