Whittington Health NHS Trust is seeking a Registered Nursing Associate (RNA) to join the MACCT team in London. This fixed-term role supports frail and complex patients, working closely with GPs, physiotherapists, social workers and other MDT colleagues to deliver proactive, holistic care.
The successful candidate will undertake frailty assessments, coordinate patient care, participate in MDT meetings and ensure prompt escalation of risks.
