UCLH@HOME is an NHS service supporting patients to leave hospital once medically optimised and safe to be managed in the community. While under this service, patients remain under the care of your consultant team. As a member of our team, your role will include direct care delivery within the patient’s own homes and the use of remote monitoring technologies. You will provide care for patients transferred to the community from UCLH prior to estimated discharge date.

The post holder will have a leadership role and take responsibility for case-finding, team coordination and organisation of transfers home. We welcome applicants from a range of backgrounds including emergency, primary care and acute medicine.
