An exciting opportunity has arisen to join our UCLH@HOME team, a service provided by Whittington Health in partnership with UCLH. We are at the forefront of integrated care – providing innovative and transformational care that best serves local communities. UCLH@HOME is an NHS service that helps patients leave hospital once medically optimised and safe to be managed in the community, while remaining under the care of their consultant team.
Responsibilities:
- Deliver direct care within patients' homes and use remote monitoring technologies.
- Provide care for patients transferred to the community from UCLH prior to estimated discharge date.
- Take a leadership role in case‑finding, team coordination, and organisation of transfers home.
Qualifications:
