Overview
North Central London TB Network is a well‑established service providing world class care to patients and their carers across five north central London boroughs. We are a nationally recognised, innovative team of nurse specialists, outreach/social care staff and administrators who work closely with the respiratory and infectious diseases teams at Whittington, North Middlesex, Royal Free and University College London hospitals. The service is based in an outpatient setting.
Responsibilities
- Work as a Tuberculosis Inclusion Caseworker with the TB multi‑disciplinary team to address the social care needs of patients with TB or suspected TB.
- Advocate for and support vulnerable populations, including the homeless, drug and alcohol users, vulnerable migrants, asylum seekers and people who have been in prison.
- Coordinate care with social services, housing departments, benefit providers, the Home Office, migrant helplines, charities and local faith groups to improve quality of care and tackle social and environmental determinants of disease.
- Develop and refine local procedures, joint protocols and community‑based provision in collaboration with other agencies and across organisational boundaries.
- Monitor and support patients’ treatment completion to help achieve the TB treatment completion rate target.
- Work independently in the community, be visible across the network and maintain a flexible approach to daily responsibilities.
Qualifications
- Motivated, energetic, efficient, and flexible with a strong desire to work for a diverse patient group.
- Comfortable working across multiple NCL hospital sites and in community settings.
- Experience or strong interest in working with homeless, drug/alcohol users, migrants, and other vulnerable groups.
- Excellent communication and interpersonal skills, able to collaborate effectively with clinical teams and community agencies.
- Knowledge of, or familiarity with, social services, housing, benefit systems and local support resources is desirable.
