The Enhanced Health in Care Homes (EHCH) West Haringey service, delivered by Whittington Health, provides proactive, multidisciplinary healthcare support to residents living in care homes across West Haringey. The service is designed to improve residents' health, wellbeing, and quality of life by delivering high-quality, personalised healthcare within care home settings and reducing avoidable hospital admissions.

The EHCH model brings together a multidisciplinary team that includes a Community Matron, Consultant Geriatrician, Pharmacist, Trusted Assessor (Nurse), Learning Disability Community Matron, and administrative support staff. The team works closely with GP practices, care home staff, residents, families, and other healthcare professionals to provide coordinated and holistic care.

The service supports residents in West Haringey care homes, including Spring Lane, The Meadow, Alexandra Park, and Fairview, while also working collaboratively with learning disability and mental health services across Haringey.

The overall aim of the EHCH West Haringey service is to ensure residents receive the right care, at the right time, in the right place, promoting independence, enhancing quality of life, reducing health inequalities, and preventing unnecessary hospital attendances and admissions.

Whittington Health serves a richly diverse population and works hard to ensure that all our services are fair and equally accessible to everyone. Nowhere is this more obvious than in the way we look after our staff. We aim to employ a workforce which is as representative as possible of this population, so we are open to the value of differences in age, disability, gender, marital status, pregnancy and maternity, race, sexual orientation, and religion or belief. The Trust believes that as a public sector organisation we have an obligation to have recruitment, training, promotion and other formal employment policies and procedures that are sensitive to these differences. We think that by doing so, we are better able to treat our patients as well as being a better place to work.
