Whittington Health NHS Trust is seeking a part-time Discharge Flow Coordinator (Band 4) to support timely patient discharge and bed flow. You will work on a ward-based role with MDTs and EIM teams, ensuring accurate discharge planning, effective communication with patients and families, and updating IT systems for real-time capacity management.
