Job Description

The post holder will work in the Haringey Rapid Response and Virtual Ward team, which is part of the wider multi-agency Haringey Urgent Care Response service supporting hospital discharges and admission avoidance. RRVW is co-located and integrated with therapy and adult social services colleagues, aiming to respond to patient referrals in the community within 2 hours to prevent hospital admission. The post holder will primarily work in Haringey borough but may be required to see patients in other NCL boroughs.

Responsibilities

  1. Provide specialist, high-quality, expert care and support for patients suitable for Rapid Response Virtual Ward (RRVW) care in liaison with multidisciplinary teams (MDT) through assessment and care planning.
  2. Initiate a range of diagnostic tests as agreed.
  3. Refer to other disciplines/professions when required.
  4. Support patients to self-manage, monitor their condition, and make informed choices.
  5. Act as a named clinician for designated patients within the service.
  6. Maintain documentation to required standards and participate in the ongoing development of the Trust\’s Electronic Patient Record system.
  7. Promote evidence-based practice development.
  8. Participate in clinical supervision to the agreed standard.
  9. Ensure high standards of care are given to all patients.
  10. Advise patients on health promotion and illness prevention.
  11. Gain intravenous access and administer prescribed medication for patients requiring short-term antibiotics, and monitor their care.
