Whitstable Medical Practice
Whitstable Medical Practice is a forward-thinking single Primary Care Network (PCN) GP Practice based across three sites in Whitstable, Kent, serving 45,500 patients. The practice employs 23 equity GP Partners, 18 Allied Health Professionals and various specialists including Frailty Practitioners, Social Prescribers, Physiotherapists and Mental Health Practitioners.
Role Overview
We are seeking a fully qualified or trainee Advanced Clinical Practitioner to join our General Practice Older Persons (GPOP) Team. The candidate should have a passion for frailty and older‑people care, completed an MSc in Advanced Clinical Practice or an equivalent registered through the Advancing Practice Academy e‑portfolio route, or be on an MSc Advanced Practice course with a completed clinical assessment and independent prescribing qualification.
Key Responsibilities
- Lead care for all local care‑home residents.
- Conduct home visits for residents with severe frailty.
- Provide urgent home visits for acutely unwell patients at risk of hospital admission.
- Arrange investigations, consider differential diagnoses, and implement management plans.
- Provide teaching and support to colleagues regarding older‑people care.
- Collaborate closely with community services such as the community frailty team and home treatment service.
- Work alongside experienced Frailty Practitioners and other Allied Health Professionals.
Qualifications and Experience
- Fully qualified or trainee Advanced Clinical Practitioner.
- MSc in Advanced Clinical Practice or equivalent via Advancing Practice Academy e‑portfolio.
- Completed clinical assessment and independent prescribing qualification (or in progress).
- Special interest in frailty and older‑people care.
- Considerable post‑registration experience.
- Professional registration required.
- Strong organisational skills, flexibility, empathy, compassion.
- Excellent communication, interpersonal skills and evidence of leadership qualities.
- Competence in assessing patients, arranging investigations, considering differential diagnoses and implementing management plans.
Additional Information
We will consider both full‑time and part‑time applications. Pay is equivalent to band 7–8, depending on experience.

In‑house contracts include: cataract surgery, ultrasound, dermatology, physical therapies, audiology. The practice also provides an urgent treatment centre, day surgery suite, digital X‑ray and various other services.
