Responsibilities

  • Implement processes for effective management of patients with long‑term conditions using evidence‑based practice, including care for elderly and housebound patients.
  • Provide multidisciplinary nursing services to the patient population.
  • Manage infection prevention and control, health promotion, chronic disease management, and well women/well man clinics.
  • Support the practice management team in reviewing and executing clinical policy and procedure.
  • Perform venepuncture, request pathology tests, and interpret results to advise patients.
  • Maintain accurate clinical notes and adhere to evidence‑based practice and PGDs policies.
  • Identify scope of practice, refer appropriately to GPs, and chaperone procedures.
  • Administer routine, childhood, and travel immunisations.
  • Provide chronic disease management for Asthma, COPD, and Diabetes.
  • Advise on travel medicine.
  • Apply broad knowledge of clinical governance and public health issues in the local area.
  • Raise awareness of wider health arena issues and implement health promotion strategies.
