Wheatley Care is seeking a health and wellbeing coordinator for our Personalised Self-Directed Support Service – Mental Health in Glasgow. You will join a team delivering high-quality, person-centred support to adults and older people with mental health needs and related vulnerabilities, guiding them to live independently at home.

You will be a keyworker, co-producing personalised plans, tracking progress with Outcome Star, and working within a trauma-informed framework to promote independence.
