This vital role involves providing essential support to individuals within the community, particularly those facing challenges related to mental health, learning disabilities, or other complex needs. Responsibilities
- Providing direct support to clients in their own homes and within the community, assisting with daily living tasks.
- Developing and implementing personalized care plans in collaboration with clients, families and other professionals.
- Offering emotional support, encouragement and practical assistance to clients managing mental health conditions or other challenges.
- Assisting with medication administration and monitoring as per care plans.
- Supporting clients in engaging with community activities, appointments and social opportunities.
- Maintaining accurate and detailed records of client progress, support provided and any incidents.
- Liaising effectively with family members, healthcare professionals and other agencies involved in client care.
- Promoting independence, choice and dignity for all clients.
- Responding to emergencies and challenging situations with professionalism and care.
- Participating in regular supervision and training sessions to enhance skills and knowledge.
- Advocating for clients' needs and rights within the community.

We are looking for individuals with excellent interpersonal skills, empathy and a genuine desire to make a difference.
