Our client, a leading provider of social care services, is seeking compassionate and dedicated Community Support Workers to join their team in London, England, UK. This vital role involves providing essential support to individuals within the community, particularly those facing challenges related to mental health, learning disabilities, or other complex needs. You will play a crucial part in empowering individuals to live independent and fulfilling lives, promoting well-being and social inclusion.

Responsibilities

  • Providing direct support to clients in their own homes and within the community, assisting with daily living tasks.
  • Developing and implementing personalized care plans in collaboration with clients, families and other professionals.
  • Offering emotional support, encouragement and practical assistance to clients managing mental health conditions or other challenges.
  • Assisting with medication administration and monitoring as per care plans.
  • Supporting clients in engaging with community activities, appointments and social opportunities.
  • Maintaining accurate and detailed records of client progress, support provided and any incidents.
  • Liaising effectively with family members, healthcare professionals and other agencies involved in client care.
  • Promoting independence, choice and dignity for all clients.
  • Responding to emergencies and challenging situations with professionalism and care.
  • Participating in regular supervision and training sessions to enhance skills and knowledge.
  • Advocating for clients' needs and rights within the community.
  • Ensuring adherence to all relevant policies, procedures and safeguarding regulations.

We are looking for individuals with excellent interpersonal skills, empathy and a genuine desire to make a difference. Previous experience in a care or support role, particularly with mental health or learning disabilities, is highly desirable. You should possess strong communication and listening skills, patience and the ability to remain calm under pressure. A willingness to undertake training and achieve relevant qualifications (NVQ/QCF Level 2 or 3 in Health and Social Care) is essential. A full UK driving licence may be advantageous depending on the specific role requirements. This role requires working shifts, including evenings and weekends.
