Practice Diabetes Nurse in Weymouth

We are looking for a practice diabetes nurse to join our friendly, supportive and progressive practice on a fixed term basis to cover maternity leave. The post will focus on providing diabetes care to our patients and up to 15 hours per week are available. The successful applicant will work as a key part of the clinical team and must have experience of working with patients with Diabetes.

Salary: Placement within this salary scale is negotiated based on skills and experience - £20.44-£24.61 per hour; £39,960 - £48,113 for 37.5 hours per week (pro rata) with the benefit of an NHS or NEST pension.

Hours: Up to 15 hours per week (with a minimum of 10 hours per week). Exact hours and working pattern can be negotiated.

Main duties of the job:

• With appropriate training, participate in the annual care needs of patients with known long-term conditions based on the quality outcome framework (QOF). This will include diabetes.
• Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care.
• Manage long term conditions at various levels of competency: Competent nurse, Experienced or proficient nurse, Senior practitioner or expert nurse.
• Support the practice to achieve the General Medical Services contract by ensuring that Quality Outcome Framework (QOF) prompts are addressed and documentation includes appropriate Read Codes.
• Promote and participate in integrated multi-disciplinary team working whereby colleagues from health and social care communicate and work in collaboration.
• Liaise and make appropriate referrals to the wider MDT and attend multi-disciplinary team meetings and events within the community.
• Ensure appropriate knowledge of other local support services and ways of referring for example healthy living, and carers support.

About us: Weymouth Bay Medical Practice is a large friendly GP practice in Weymouth, Dorset. The Practice cares for 26,500 patients with 3 purpose-built surgeries. We are a training practice with a dedicated team of 22 GP Partners and 60 nursing and administrative staff, and we value the contribution of each staff member.

Job responsibilities:

• Assess, plan, develop, implement and evaluate nursing care to promote health and wellbeing, and prevent adverse effects.
• Participate in the annual care needs of patients with known long-term conditions based on the quality outcome framework (QOF).
• Identify and manage treatment plans for patients at risk of developing a long-term condition with appropriate screening and prevention strategies.

Person Specification:

• Essential: Knowledge of needs of patients with long-term conditions, awareness of accountability of own role, knowledge of health promotion strategies, communication skills, IT skills, effective organisational and time management skills.
• Desirable: Ability to identify determinants on health in the local area, knowledge of public health issues, awareness of local and national health policy.

Qualifications:

• Essential: Membership of a professional body, registered nurse (adult), Diploma in chronic disease in particular diabetes.
• Desirable: Specialist Practitioner qualification.
