Job summary

Due to ongoing expansion of our clinical team, we are looking for an enthusiastic qualified ANP with current registration to work within our multi-disciplinary team within both the practice and also within the local community. The postholder will be an experienced, autonomous clinician who provides high-quality, patient-centred care. This exciting full-time salaried role splits time between clinic-based consultations and home visits. Patients include the housebound, frail, or complex demographic. The ANP utilises advanced physical assessment, diagnostic, and independent prescribing skills to manage undifferentiated presentations.

Main duties of the job

You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat, or refer patients.

  • Carry out consultations with patients taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis.
  • Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate,
  • Provide home visits to our housebound, care home and complex patients .

About us

Wetmore Road Surgery have a patient population of approximately 12,500 and are led by six GP Partners in a supportive and caring environment. The practice operates from a busy town centre site with dedicated staff car parking.

We have a large, multidisciplinary clinical team comprising GPs, Advanced Nurse Practitioners (ANPs), Practice Nurses, Healthcare Assistant (HCA), and Pharmacists. We also have a dedicated Outreach Team who support our more vulnerable patients within our local community.

We are an accredited training practice and are committed to supporting the education and development of healthcare professionals. We actively encourage and support the ongoing professional development of all members of our team.

Job responsibilities

Key Responsibilities

1. Clinical Practice & Home Visiting

  • Autonomous Consultations: Manage a clinical caseload using face-to-face, telephone, and digital triage pathways.
  • Domiciliary Care: Undertake acute and routine home visits for housebound or complex patients.
  • Clinical Assessment: Conduct detailed history-taking and physical examinations of undifferentiated conditions.
  • Diagnosis & Management: Formulate accurate diagnosis and treatment plans for acute illness and chronic diseases.
  • Independent Prescribing: Prescribe safely and effectively within current legal and local frameworks.
  • Investigations: Order, interpret, and action diagnostic tests including pathology and radiology.
  • Escalation: Make referrals to secondary care, community teams, or arrange emergency admissions when required.

2. Care Quality & Administration

  • Documentation: Maintain accurate, comprehensive electronic patient records using EMIS Web.
  • Coding: Ensure effective use of SNOMED CT coding for clinical information.
  • QOF & Targets: Support the practice in hitting targets related to QOF and local health contracts.
  • Audits: Participate in and lead clinical audits to maintain evidence-based local guidelines.

3. Leadership & Education

  • MDT Collaboration: Work with GPs, Outreach Team and District Nurses, to coordinate care pathways.
  • Mentorship: Provide clinical supervision and support to practice nurses and junior staff.
  • Professional Development: Maintain professional revalidation in line with NMC standards.

Person Specification

Qualifications

Essential

  • Current professional registration with the NMC, HCPC or other relevant professional body.
  • Holds the recognised qualifications and credentials required to meet the NHS England Advanced Practice framework and be eligible for an Advanced Practice Digital Badge (where applicable to the role).
  • Master's level qualification in Advanced Clinical Practice (or equivalent)
  • Independent Prescribing qualification.
  • Evidence of continuing professional development

Desirable

  • Master's degree in Advanced Clinical Practice.
  • Teaching, mentorship or assessor qualification.

Experience

Essential

  • Experience working as an Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) within Primary Care, Urgent Care or Emergency Care
  • Experience of autonomous clinical assessment and decision making.
  • Experience managing acute and minor illness presentations.
  • Experience managing frail/elderly patients and those with long-term conditions.
  • Experience of multidisciplinary team working.

Desirable

  • Previous experience in General Practice.
  • Experience using EMIS Web.
  • Experience undertaking care home rounds.
  • Experience of telephone and online triage.
