We are excited to be able to offer an opportunity to join our team as an out of hours care co-ordinator. This is a fantastic opening for an individual who is hungry for a new challenge with a company that will support you to develop your career.
You will be joining a privately owned organisation, which offers a range of care services to older people and children within their own homes. With a fantastic ethos and dedicated team, passion for quality care is cascaded throughout the organisation. This passion runs from the boardroom to the ground. You will be mentored in your role by a dedicated management team who will help you to succeed and develop.
Experience in an on-call role is not essential as full training will be given. However, at least 2 years care experience is essential. If you are passionate about the delivery of high standards of care and support in people's own homes, have experience in a social care setting, and are able to prioritise and problem solve using your own initiative, you may be the person we are looking for.
Duties will include:
- Ensure the safe and effective delivery of care and support to all Service users outside of office hours
- Work evenings and alternate weekends, Bank holidays on a rota basis
- Maintaining effective, positive relationships with a variety of people including service users, members of the organisation, families and other professionals
- Receiving out of hours calls from service users and their families, colleagues, and other professionals
- Responding appropriately to their requirements
- Resolving issues that may arise and collate accurate notes
- Taking actions in line with the organisations policies and procedures
- Deliver care and support to service users at short notice to meet the demands of the service
- Provide accurate information during daily hand overs to ensure a seamless service
- Monitoring our call alert system to ensure that all Service Users have received their care call and staff have arrived safely and on time

Requirements:
- Must hold a full UK driving license
- Must hold NVQ level 2 or above in Health and Social Care and be willing to work towards the Diploma Level 3 in Health and Social Care
