Westrop Medical Practice is seeking an experienced Advanced Clinical Practitioner to join our clinical team and undertake autonomous clinics as part of the GP rota. Westrop is a large, multi-site GP partnership serving approximately 49,000 patients across six sites in North Swindon. Our multidisciplinary team includes GPs, nurses, pharmacists, paramedics, physiotherapists and care coordinators working together to manage both same‑day demand and ongoing care.

This role is suited to a confident ACP who is comfortable managing undifferentiated presentations, making independent clinical decisions and contributing to the day‑to‑day management of acute and routine GP workload. The successful candidate will run their own clinics within the GP rota, assessing, diagnosing and managing patients, arranging investigations, prescribing where appropriate and coordinating care with the wider team. You will be supported by an experienced clinical team and allocated a GP mentor, ensuring consistent clinical support, opportunities for discussion and ongoing professional development.

Main duties of the job

The Advanced Clinical Practitioner will undertake autonomous clinical assessment and management of patients as part of the GP rota, working across Westrop sites as required. The role will include:

  • Assessing and managing patients presenting with undifferentiated and undiagnosed conditions.
  • Undertaking clinical examinations, formulating differential diagnoses and implementing appropriate management plans.
  • Prescribing where appropriate, arranging and interpreting investigations and making referrals to secondary care or community services.
  • Providing safe, effective and patient‑centred care.
  • Maintaining accurate and timely clinical documentation, coding and completion of administrative tasks.
Job responsibilities
  • Undertake autonomous patient consultations within the GP rota across Westrop sites as required.
  • Take comprehensive clinical histories and perform physical examinations.
  • Formulate differential diagnoses and develop appropriate management plans.
  • Prescribe medication where appropriately qualified to do so.
  • Arrange and interpret investigations including blood tests, imaging and other diagnostic procedures.
  • Refer patients to secondary care, community services or other healthcare professionals as appropriate.
  • Provide health advice, preventative care and patient education.
  • Manage both same‑day and routine clinical workload.
Clinical Governance and Patient Safety
  • Work within professional scope of practice and competence.
  • Maintain safe and effective clinical decision‑making at all times.
  • Escalate complex cases or uncertainty to GP colleagues where appropriate.
  • Participate in clinical audits, quality improvement initiatives and practice governance activities.
  • Adhere to national and local clinical guidelines and protocols.
  • Support the practice in maintaining high standards of patient safety.
Administrative and Clinical Record Keeping
  • Maintain accurate and contemporaneous patient records using SystmOne.
  • Ensure appropriate clinical coding and documentation of consultations.
  • Complete administrative duties associated with patient care, including results management, prescriptions, correspondence and follow‑up actions.
  • Contribute to effective management of workflow tasks related to patient care.
Multidisciplinary Team Working
  • Work collaboratively with the wider multidisciplinary team including GPs, nurses, pharmacists, paramedics, physiotherapists and care coordinators.
  • Participate in clinical meetings and case discussions.
  • Contribute to a supportive and collaborative clinical environment.
Professional Development
  • Maintain professional registration and comply with continuing professional development requirements.
  • Engage in regular clinical supervision and mentorship with an allocated GP mentor.
  • Participate in relevant training and educational activities to maintain and develop clinical competence.

Working Relationships

The postholder will work closely with GP Partners and salaried GPs, nursing team, pharmacists and pharmacy technicians, paramedics and physiotherapists, practice management team, and administrative and reception staff.

Person Specification
  • Registered healthcare professional with NMC, HCPC or equivalent professional registration.
  • MSc in Advanced Clinical Practice (or equivalent experience working at ACP level).
  • Significant post‑qualification clinical experience.
  • Experience assessing and managing patients with undifferentiated and undiagnosed conditions.
  • Ability to undertake autonomous clinical consultations and make independent clinical decisions.
  • Experience managing acute presentations and same‑day demand.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively within a multidisciplinary team.
  • IT literacy and experience using electronic patient record systems.
  • Experience working within primary care or a GP practice setting.
  • Experience working within a GP rota or same‑day demand service.
  • Experience using SystmOne clinical system.

Qualifications
  • Registered healthcare professional (NMC / HCPC or equivalent professional body).
  • Qualified Advanced Clinical Practitioner (MSc Advanced Clinical Practice or equivalent experience).
  • Independent prescribing qualification (or working towards).
  • Significant experience working in primary care or urgent care settings.
  • Experience assessing and managing patients with undifferentiated conditions.
  • Commitment to continuing professional development and clinical governance.
  • Experience supervising or supporting junior clinical staff.
  • Experience participating in clinical audit or quality improvement projects.
  • Evidence of postgraduate training relevant to primary care.
  • Teaching or mentoring experience.
  • Understanding of local referral pathways and NHS primary care systems.
