About the Role
Being a Care Coordinator with Westmorland Homecare is a very special job and you will have the chance to make a difference to people’s lives within your community. It is an opportunity to build friendships that last a lifetime and create treasured memories for both you and our clients. The role is based in our office Monday to Friday, 9 am‑5 pm, with an on‑call rota that includes evenings and weekends. Being a driver is essential for the role. Our care coordinators are allocated a specific area of responsibility and, by working closely with each other, provide day‑to‑day support for our care team.

Pay: £29,000.00 per year
Duties
- Answer incoming calls in a positive, happy, professional, unrushed and pleasant manner, making the caller feel important and special
- Complete needs assessment for new clients
- Write care plans for new clients and keep them up to date
- Introduce HCAs to new clients and ensure they understand the clients’ needs and wishes
- Complete all client quality assurance checks as per the QA schedule
- Complete all client service reviews and care plan revisions as per the QA schedule
- Follow up any actions required as identified in QA and service review
- Do interim service reviews when required due to changes in needs, hospitalisation, etc.
- Send referrals to other allied agencies when the need is identified by any staff member
- Build positive collaborative relationships with our partner organisations such as OTs, DNs and GPs
- Ensure all our clients are happy and have a great experience of their care
- Contribute to staff retention
- Participate in a share of the on‑call rota out of hours
- Promote the good name of Westmorland Homecare in the community
- Carry out any other activities that may be necessary to meet the needs of the business
Qualifications
- Proven office experience in a client‑facing role
- Having a driver’s licence and access to your own car is essential for the role
- Strong understanding of care plans
- Excellent communication and interpersonal skills
- Ability to coordinate and manage multiple tasks efficiently
- Detail‑oriented with strong organisational skills
- Experience in healthcare or related fields is a plus
