As an Intermediate Care Support Worker you will work with people who have been identified as having intermediate care needs, providing support in an empowering way, to promote recovery and maximise their independence. This is an incredibly rewarding opportunity, giving you the chance to work with people on achieving identified goals and outcomes, supporting them to retain as much independence as possible and remain living within their local community.
Responsibilities
- Supporting individuals with personal care, ensuring individuals are treated with dignity and respect.
- Supporting with, and administering medication as required.
- Working with individuals on their own personal outcomes, as agreed within a person centred, strength-based plan.
- Working closely with colleagues across the Service, including Therapists and Social Workers.
