An exciting opportunity has arisen for a highly motivated individual to work as part of our brokerage team. Brokerage is a key part of the successful delivery of Adult Social Care. If successful you will support the delivery of excellent services to a wide range of internal and external stakeholders such as: People who use adult social care services, social workers, commissioners, providers of adult social care and health partners.
The role will include:
- Supporting people with eligible adult social care needs to access the services they need by working with providers of adult social care.
- Working with colleagues to ensure peoples’ identified care needs are met in the most appropriate way.
- Supporting the brokerage service to contribute to ensuring the delivery of care is effective and safe.
- Making sure information is recorded accurately and in a timely manner to ensure quality management information and essential data is up to date.
- Building effective relationships with a variety of stakeholders.
- Using a range of specific IT systems in order to carry out the functions of the role.
