We are seeking a Housing Specialist for our Permanent Supportive Housing Department.

Responsibilities & Duties:

· Assists participants with obtaining household goods, utilities, and other move-in necessities.

· Develops individualized housing stabilization plans, coordinates admissions and discharges from inpatient treatment.

· Collaborates and liaises with law enforcement officers, local hospitals, medical providers, and others in the planning, provision, and evaluation of these services.

· Participates in daily PSH team meetings to discuss client treatment goals and needs.

· Collaborate with other members of the PSH Team to provide comprehensive mental health and supportive services to clients.

· Provides up-to-date documentation consistent with agency policies and procedures.

· Receives clinical and procedural guidance from the PSH Program Manager.

· Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.

Qualifications and Skills:

Bachelor’s degree in a relevant field with professional, clinical experience in the provision of substance abuse treatment or a combination of education, training, and experience indicating the possession of essential knowledge, skills, and abilities required.
