Are you an experienced Senior Case Manager or an experienced Case Manager looking to step up? We have an amazing opportunity for a Senior Case Manager to join us. This is an exciting opportunity for a qualified registered professional with at least 3 years’ case management experience to join us. Westcountry Case Management is an award-winning case management service with a CQC Outstanding rating in all 5 areas as well as being registered with the CIW. 2026 is our 25th anniversary, and an exciting time to be joining us!
Westcountry Case Management are a leading case management service supporting people in reaching their goals, maximising their potential and quality of life. We are long established, widely regarded, and highly trusted in the Westcountry and Wales regions. We operate in the independent sector, and the company is part of the Active Care Group.
You will retain a clinical case load as well as providing support to the Business Director and Clinical Lead with regards to clinical procedures and processes. In addition, you will take on supervisory responsibility for other case managers within the company and play a large part in inducting new case managers. Westcountry Case Management provides case management to both adults and children and young people. We work with clients with catastrophic injuries, with a specialism in ABI. We also have clients with other complex conditions. We are registered with CQC & CIW to enable our clients to directly recruit their own support workers.
What you'll be working:
- Monday to Friday 37.5 hours per week
What you'll be doing:
- Provide supervision to case managers in line with the company Supervision Policy.
- Provide additional clinical support to their designated supervisees as reasonably required, between formal supervision sessions.
- Undertake induction programme for new case managers.
- Signpost individuals to others who may have more relevant experience, or time, to assist with a particular clinical issue, if required.
- Undertake Quality Assurance/audits on their supervisee’s case management progress reports, and any other complex clinical documentation as required.
- Provide feedback and constructive support to optimise standards.
- Report any concerns to the Clinical Lead/management team.
- Provide information to the Business Director on the capacity of their supervisees to take on additional cases.
- Alert Clinical Lead of any concerns in relation to the clinical competency or professional conduct of their supervisees.
- Undertake appraisals of their supervisees.
- Attend and contribute to clinical meetings as required.
- Attend and contribute to business development with the Business Director.
- Assist Clinical Lead with clinical processes and procedures, writing policies and processes as appropriate and with support of the Clinical Lead.
- Assist Registered Manager with regulatory oversight input as appropriate.

What you'll have:
- Relevant qualification in nursing, social work, occupational therapy, speech & language therapy or physiotherapy.
- Current registration with the appropriate professional body e.g. SWE, HCPC, NMC.
- BABICM Registered (Advanced BABICM is preferable).
- Minimum 7 years post-qualification experience with at least 3 years case management experience.
- Experience of working with clients who have brain injury, neurological and other complex conditions.
- Successful candidates will be required to undergo an Enhanced DBS.
