The predicted salary is between 37000 - 44000 £ per year.
As a Clinical Pharmacist, you will play a key role in supporting the management of long‑term conditions, medication optimisation, structured medication reviews and medicines reconciliation.
You will work closely with GPs, nurses, care coordinators and other healthcare professionals to improve prescribing quality and patient safety.
Run independent long‑term condition clinics eg; hypertension, diabetes, cardiovascular disease, asthma/COPD and other agreed areas.
Assess, diagnose within scope, prescribe and manage patients independently.
Manage medication authorisations, repeat prescribing and complex prescribing queries.
Support QOF, IIF and Enhanced Service delivery.
Lead medicines optimisation, prescribing safety and high‑risk drug monitoring.
Undertake clinical decision‑making for complex patients in a busy primary care environment.
Use population health management and risk stratification to identify patients for proactive review.
Support care home, frailty and anticipatory care.
Lead audits, quality improvement and prescribing projects.
