Are you passionate about providing exceptional care and support to individuals living with dementia? West Berkshire Council is seeking dedicated and compassionate Dementia Practitioners to join our Adult Social Care Team, working within the Birchwood Care Home offices. Reporting to the Registered Manager, you will play a vital role in delivering high‑quality, person‑centred care to our residents.
About the Role
- Supporting the Registered Manager in the day‑to‑day operation of the care home
- Ensuring delivery of high‑quality, person‑centred care to individuals living with dementia
- Implementing and maintaining accurate care plans, regularly reviewing and updating them in collaboration with residents and their families
- Monitoring and assessing the physical and emotional well‑being of residents, providing appropriate support and intervention
- Working collaboratively with a multidisciplinary team, including healthcare professionals, to ensure comprehensive care delivery
- Providing guidance and support to care staff, promoting best practices in dementia care
- Contributing to regular team meetings and participating in care reviews
About You
- Experience working in a similar role within a care home environment
- In‑depth knowledge and understanding of dementia and its impact on individuals
- Excellent communication and interpersonal skills, with the ability to build positive relationships with residents, families, and colleagues
- Strong observational and assessment skills, with the ability to identify changes in residents\' needs
- Ability to provide guidance and support to care staff, fostering a culture of learning and development
- Knowledge of relevant legislation and regulatory requirements, including CQC standards
- A compassionate and empathetic approach, with a genuine desire to improve the quality of life for individuals living with dementia
