Your excellent communication and interpersonal skills enable you to interact effectively with a range of stakeholders, while your IT proficiency ensures effective record-keeping. With a valid full driving license, you are ready to travel across the county.

What the Job Involves

Working within West Sussex County Council's Countywide review service for Adult Services, you will complete strengths-based assessments and reviews for adults 18+ with diverse needs, aiming to improve their wellbeing and independence. The role requires conducting mental capacity assessments and using Continuing Health Care tools, as part of a dynamic social work team. Beyond completing assessments, you will develop and apply support plans, collaborating with individuals, families, and carers, aligning interventions with the needs and strengths of the people served. You'll provide informal supervision for less experienced practitioners and contribute to team strategies and solutions.
