Working arrangement: Full Time
Mixture of home and hospital work, attending hospital 4 days/week for assessments, family meetings, and ward visits. Also visiting clients at home for assessments. Part of a multidisciplinary team covering A&E and emergency floor.
Pay rate: £35/hour, capped by MoC. Rates up to £38/hour possible for those with at least 2 years qualified experience. Candidates should be aware that the offered rate may be less than requested based on skills and competency. Additional expenses of £150/week may be available for candidates living outside West Sussex.
Key Responsibilities: Conduct Care Act and Mental Capacity assessments, support planning, undertake safeguarding, participate in duty team activities, and attend MDT meetings. Respond promptly to assess hospital and discharged patients’ needs, facilitating care and support to maximize independence. Opportunities to collaborate with various professionals and develop as practitioners.
