About the Role
Working under the direct supervision of the lead nurse, you will assist the clinical team in the provision and delivery of prescribes programmes of patient care.
The main responsibilities of the role include:
- Responsibility for meeting the nursing care needs of patients identified by the nurse, or where care has been transferred to the nurse by the GP.
- Triage of patients seen in the treatment room, or by telephone; offering advice, nursing treatment and/or emergency or routine referral to the doctors, or specialist nurse, or other agencies if appropriate.
- Responsibility for identifying care needs within the practice population, by practice profile or audit; and organising work to best meet the needs identified.
- Managing nursing care of those patients with chronic disease, operating specific clinic sessions if needed, and maintaining an appointment and recall system for patients.
- Involvement in policy development within the practice, providing the nursing input in any decision-making processes affecting the care of patients.
- Teaching other practice nurses and other members of the practice nurse team (nursing assistants/phlebotomists).

About the Candidate
- Registered General Nurse (RGN).
- Minimum of 2 years post registration work in community or general practicenursing.
- Competence to undertake asthma, COPD and diabetic reviews.
- Evidence of clinical skills and experience in treatment room consultation, screening, management of long-term conditions, vaccinations and immunisations, travel health, contraception, cervical screening and wound care.
- Patient focussed.
- Able to work under minimum supervision.

We have 11,500 patients on our list, a CQC rating of βGoodβ and operate from both our main site and a branch site in Newton Flotman.
