About the Role

East MK PCN is seeking a permanent Care Coordinator to join the team on a full-time basis. The Care Coordinator will support the delivery of proactive, personalised care to patients with frailty by working closely with the Integrated Community Support (ICST) and wider multidisciplinary team (MDT). The role will involve assisting with care planning appointments, undertaking defined elements of patient interaction, and completing associated administrative tasks to ensure accurate, high-quality care plans are developed, recorded, and reviewed.

Responsibilities

  • Assist in the coordination and delivery of care planning appointments for frail and complex patients
  • Prepare patients for care planning discussions
  • Undertake agreed parts of care planning appointments (e.g. gathering patient information and goals)
  • Document care plans accurately within the clinical system
  • Act as a first point of contact for patients and carers
  • Work with ICST and MDT members to deliver coordinated and patient-centred care
  • Book appointments and maintain patient records
  • Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date
  • Create reports when needed

About the Candidate

If you are a skilled care coordinator, with excellent person-centered planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

Qualifications

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups
  • Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential)
  • Familiar with local resources and services, including how to access them
  • Motivated by helping people, with care and empathy
  • Able to deliver person-centred support in a non-judgmental way
  • Experience of completing holistic person-centred care planning assessments
  • Strong listening and communication skills
  • Ability to work with frail or vulnerable patients
  • Good administrative and organisational skills
  • IT competency (clinical systems, MS Office)

Additional Information

The post-holder may be required to work across multiple sites within the PCN. Training and supervision will be provided.
