Hillton PCN is seeking an experienced advanced nurse practitioner to join our team on either a full or part-time basis. The post-holder will be required to act within their professional boundaries, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for the patients within the general practice. The post-holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met.
Main Responsibilities
- Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth, theoretical knowledge-base.
- Assessing, diagnosing and treating patients' conditions, initiating direct referrals, as appropriate.
- Providing expert advice to patients, carers and colleagues.
- Developing innovative concepts, models, methods and practices, to help deliver improved primary care to our practice population.
- Advancing patient health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
- As an independent prescriber: to prescribe safe, effective and appropriate medication, as defined by current legislative framework.
- Leading and undertaking research and audits.
- Providing leadership, education and training to different healthcare professionals, staff and students.

About the Candidate
- Registered general practice nurse, holding current registration with the NMC.
- Hold a recognised ANP qualification at master's level.
- Hold an independent prescriber qualification.
- Work with a high degree of autonomy.
- Confident in leading in a variety of settings; our leadership of others will also enable you to use and further develop your teaching and mentoring experience/qualifications.
- Forward-thinking and will strive to develop innovative ideas and new ways of working, that address the health needs of the practice, whilst motivating and encouraging others to do the same.
- Excellent communicator and collaborator.

About Us
Serving approximately 33,000 patients, across Ampthill and Barton-Le-Clay, Hillton PCN strives to develop collaborative working across the three member practices, aiming to support and strengthen the delivery of primary care services in line with the NHS long-term plan. We aim to achieve high standards of patient-centred care as identified for the specific needs of our population, whilst working to support a resilient and appropriately trained workforce. We operate SystmOne clinical system with a good history of QOF achievement and a CQC rating of 'good' across all domains. We currently have a paramedic, three clinical pharmacists, a care coordinator, a social prescriber, two first contact physiotherapists and a primary care network manager. We are also recruiting for further roles to support the future of the network.
