About the Role
EAST:MK Primary Care Network has an exciting opportunity for a health and wellbeing coach to join our practice team, on a full-time basis, for 37.5 hours per week. You will be working over 5 days, between Monday and Saturday, on a rotation basis across our 4 surgeries; Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery, that make up the PCN.
You will work collaboratively with the multidisciplinary team (MDT) within the PCN, ensuring all patients receive the best possible care and service. The focus of the role is to support our patients to feel in control of their life and make the necessary behavioural changes to work towards their personal goals.
You will be coaching and motivating patients through multiple sessions, to identify their needs, set goals and support them to implement their personalised health and care plan to ensure that they are active participants in their own healthcare, empowering them to take more control in managing their own health and well-being, to live independently and to improve their health outcomes.
Responsibilities of the role will include:
- Care-planningand the delivery of systematic self-management support, based on the knowledge of everyone’s activation levels.
- Care-coordination across the care continuum (including identification and support of carers).
- Supporting effective team-working in primary care, through taking on appropriate practice-based tasks, attending regular team meetings, if required, and working with the complex-care teams.
- Delivering group and one-to-one sessions on healthy eating, physical activity and health and wellbeing, to tie-in with personalised care plans.
- Engaging patients, and using health-coaching techniques, to support them to take an active role in their health and wellbeing.
- Supporting practices where necessary, including NHS Health Checks, Smoking Cessation, and outreach programmes.
- Helping to raise the local population’s awareness of the support groups and the opportunities available to assist them in achieving their health and wellbeing goals.
- Working in partnership with clinical and non-clinical colleagues, management support and the wider PCN, to ensure the role delivers the best possible outcomes for our patients.
About the Candidate
- Experienced health and wellbeing professional,confident to deliver motivation, support, and advice to a range of patients with diverse health needs.
- Be kind, reflective and self-aware who recognises what matters to patients.
- Knowledge in dealing with patients who have long-term conditions and a lack of knowledge, or skills, to improve and manage their own health and wellbeing effectively.
- Experienced in delivering personalised care plans.
- Confident to write concise, accurate and timely bespoke care assessments, alongside a multi-disciplinary team, to improve patient health across our community.
- Adept in encouraging patient activation.
- Completed a four-day PCI accredited health coaching training course or be willing to complete one prior to taking referrals.
- Completed Tier 1 level of practice/expertise for health and wellbeing coaches or willing and supported to do so.
- Excellent interpersonal and communication skills.
- Strong local knowledge, to promote local support networks and services in our community.
Desirable criteria:
- Coaching, counselling qualification, experience or other relevant qualification/experience involving reflective listening skills.
- Relevant qualification in health and wellbeing/mental health or physical activity.
- Possess experience of health promotion or health behaviour change work with individuals and community groups.
- Experience in health and nutrition work with patients.
- Experience of working as a health advisor or trainer
- Experience working on SystmOne (or similar clinical systems)
- Good understanding on social prescribing and community development setting.
