About the Role

Haven Health are seeking to recruit an enthusiastic and flexible Practice Paramedic to work in the community treating both Haven Health and Howard House patients. The vacancy is for approximately 30 hrs a week, over 4 days; however, flexibility is crucial to accommodate training, holidays and occasional sickness. Suitable candidates must be able to work flexibly and autonomously in the community supporting both our housebound and care home patients. It is not necessary to hold a prescribing qualification for this role but it is necessary to be a proactive team member.

Duties will include:

  • Treat both housebound and care home patients across 2 surgeries
  • Lead on the various vaccination campaigns
  • Complete referrals where applicable
  • Monitor long term conditions and work closely with GPs/debriefing following home visits
  • Ensure all medical notes are written contemporaneously and succinctly
  • Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs in the patients' homes/care homes
  • Act as an autonomous clinician, with the support and guidance from the GPs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan
  • Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history
  • Recognise, assess and refer patients presenting with mental health needs
  • Telephone triage as appropriate for clinics and home visits and care home ward rounds and provide access to GPs as necessary
  • Evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral, including admission to hospital if required
  • Signpost appropriately to other services in the identified healthcare pathway and refer patients to appropriate care settings, within locally agreed healthcare pathways
  • Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events
  • Advise and support members of the public on healthy lifestyles, health promotion and disease prevention
  • Provide clinical advice to other members of the team where appropriate
  • Possibility to work in the clinic setting as required
  • Any other duties within the scope of the post holder as requested by the management team

About the Candidate

If you are an experienced emergency care practitioner (ECP)/paramedic wanting a new challenge, we would love to hear from you. You will have the following skills and experience:

  • Be a registered paramedic, with current HCPC registration and have post-registration experience, ideally with some being in primary care
  • Desirable: Minor illness, Phlebotomy, SystmOne trained
