About the Role
Ranworth Surgery, in Clacton, is seeking an experienced practice nurse to work on a part-time, permanent basis for 13.5 hours per week, to support the provision of high-quality healthcare and a diverse patient list. Hours will be: Monday 08.30 - 18.30 (with 1 hour lunch break) Tuesday 08.30 - 13.00.
Working as a senior member of the practice multidisciplinary team, the post holder will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients.
Main responsibilities include:
- Support the production of the IPC annual statement in conjunction with the nurse manager.
- Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
- Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
- Develop, implement and embed health promotion and wellbeing programmes.
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health.
- Diagnose and manage acute and chronic conditions.
- Review patient medications to enhance compliance.
- Support the clinical team.
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
Essential
- Registered nurse – current NMC registration with an active PIN.
- Experience working in a primary care environment.
- Chronic disease management experience.
- Infection and preventative measures experience.
- Experience of continuing professional development (CPD).
- Flexible to working outside office hours.
Desirable
- Mentor or teaching qualification.
- Nurse prescriber.
- Postgraduate diploma or degree (chronic disease management).
- Family planning qualification.
- Experience of working as a community nurse.
