About the Role
- Provide skilled nursing intervention/care to patients of any age or condition in partnership with the community nurse and the clinical direction of the general practitioners at the practice.
- Undertake delegated duties under the supervision of the community nurse team leader or senior on duty.
- Take responsibility for the caseload in the absence of the team leader including new visits/assessments, workload management and allocation, dealing with referrals etc.
- Maintain close liaison with the community nurse team leader, the team, GP and other members of the primary care team and wider health and social care team.
- Apply current infection control policy issues and universal precautions.
- Encourage patients towards self care and independence.
- Maintain up to date knowledge of methods of rehabilitation.
- Demonstrate, teach and supervise the nursing care that families/carers can safely undertake.
- Ensure that adequate medical supplies, equipment and other materials are maintained.
About the Candidate
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Previous experience working as a practice or community nurse.
- Experience of working in a primary care environment.
- Experience of chronic disease management, infection control, rehabilitation methods.
- Able to work autonomously.
- Assisting with quality initiatives, i.e. benchmarking.
