The post-holder will add capacity and expertise to improve patient care within the practice and work closely with clinicians and external professionals to deliver holistic care.
The role involves coordinating MDT meetings, organising referrals, promoting health checks and immunisations, and supporting safeguarding and SAR processes across the PCN.
