Our Reablement Support Officers work with a number of people in the community each day. They travel to residents' homes to support them with regaining skills and confidence to enable their independence, actively promoting choice and dignity. The role is rewarding – each visit offers a new challenge. Officers work autonomously within a close‑knit team and receive regular support from the Reablement Team Manager, in partnership with other health teams such as the intermediate care team.
Shift Pattern
- Cycle of 4 days on followed by 4 days off.
- Within each 4‑day block:
- 2 Morning Shifts: 07:00‑11:30 (16‑hour total) or 07:00‑16:00 (31.5‑hour total)
- 2 Evening Shifts: 18:00‑22:30 (16‑hour total) or 13:30‑22:30 (31.5‑hour total)
Total weekly hours: 16 or 31.5, with some weekend and bank holiday shifts required.
Pay
£14.88‑£15.68 per hour, depending on experience, plus mileage reimbursement of 45p per mile.
Key Responsibilities
- Visit multiple individuals in their homes, primarily working independently.
- Support day‑to‑day activities including personal care, meal preparation, mobility, and community access to help patients regain independence.
- Monitor progress and maintain case records throughout the reablement journey, from assessment to discharge.
- Assist clients in establishing and reviewing individual reablement goals.
- Refer clients to appropriate agencies such as Assistive Technology, Age UK, Northamptonshire Carers, and others when needed.
Qualifications & Requirements
- Previous experience in a health or social care setting.
- NVQ Level 2 in Health & Social Care or equivalent, or willingness to work towards it.
- Full driving licence and personal vehicle, capable of county‑wide travel within Northamptonshire.
