Our Review Team plays an important and specialist role in ensuring that people already receiving care continue to have the right support in place. This is not your standard community based role. You will primarily work with individuals aged 65 and over who are living in residential and care home settings, taking a proactive approach to reviewing long term care arrangements and ensuring they remain appropriate and sustainable.

As an Adult Social Care Worker within the Review Team, you will manage a varied caseload and build strong, ongoing relationships with the people you support. Reviews can take place over several months and sometimes longer, so continuity and attention to detail are key. You will work closely with families, providers, and internal teams including finance and legal, giving you exposure to a broad and rewarding area of adult social care.

You will also have the opportunity to work alongside Qualified Social Workers and develop your knowledge through shadowing more complex work, including legal processes and decision making. This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate. It is also a regulated activity and will be subject to a Children and Adults Barred List check. What will you be doing?

  • Play a key role in reviewing care placements and supporting people to receive the right care, in the right setting, at the right time.
  • Carry out planned and responsive reviews for people living in residential and care homes, ensuring care remains person centred and appropriate.
  • Work collaboratively with individuals, families and professionals to assess needs, risks and outcomes, making recommendations where care arrangements may need to change.
  • Manage your own caseload, organising and prioritising your work to ensure consistent support for individuals, including those with complex or long term needs.
  • Develop confidence around financial awareness and challenge, supporting work around financial assessments, understanding income, assets and contributions towards care.
  • Work closely with finance and debt teams where funding arrangements may be complex or under review.
  • Recognise when information may not be accurate and feel confident asking the right questions, including identifying potential financial abuse or undeclared assets.
  • Identify and respond to safeguarding concerns, ensuring appropriate action is taken where needed.
  • Maintain clear, accurate and up to date records using internal systems.
  • Build positive working relationships with partners, providers and colleagues.
  • Work alongside Qualified Social Workers, with opportunities to learn and develop through exposure to more complex casework.

About you

We are looking for someone who brings a calm, confident and thoughtful approach to their work, and who is comfortable working in situations that can sometimes be complex or sensitive.

  • Experience or a good understanding of adult social care, particularly supporting older adults or those with ongoing care needs.
  • The ability to have open and respectful conversations, including discussions around care, finances and decision making.
  • Confidence in asking questions and appropriately challenging where things do not feel right.
  • Strong communication and report writing skills.
  • The ability to manage your own workload and organise competing priorities effectively.
  • An understanding of safeguarding and a commitment to keeping people safe.

You will also have:

  • Knowledge of key legislation, including the Care Act and Mental Capacity Act.
  • The ability to assess situations, consider risk and make informed decisions.
  • Confidence using IT systems and maintaining accurate records.
  • A caring, professional and person centred approach to working with individuals and families.
  • Experience within a review style role, knowledge of continuing healthcare or experience using Liquidlogic, although this is not essential.
