As part of the West Lothian Health and Social Care Partnership (HSCP), the Care Home Collaborative team is an integrated social care team with social workers, community care coordinators, advanced nurse practitioners, band 6 nurses and a bowel and bladder specialist. The team completes initial and annual reviews of care and nursing home placements, undertakes care management tasks and has statutory responsibility under Adult Support and Protection and Adults with Incapacity legislation.
West Lothian is a progressive council and following extensive restructuring and modernisation this post sits within one of the specialist teams which operate alongside a locality-based model for the Health and Social Care Partnership. The team is fully integrated with health and social work and the post has direct line management responsibility by a social work team manager.
This is an interesting and challenging opportunity for an enthusiastic person to work with individuals with a range of complex needs. Candidates should ideally have experience of working with various client groups. The successful candidates will have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and report writing skills.
These tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals and agencies. The posts require a good understanding of a strengths based, outcome focussed approach to care planning and reviewing. You will also undertake crisis work and Adult Support.
