The successful candidate will be based within the Integrated Discharge Hub – Hospital Social Work Team and responsible for completing assessments for adults and older people to facilitate safe and supported discharges from hospital.

The Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care officers, reablement staff, discharge facilitators, AHP staff as well as other representatives from different services. The hub promotes a home first approach and works towards patients being discharged on their predicted date of discharge (PDD).

This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.

The worker must have the ability to work as part of a team and respond to the demands of a busy service, all in a pro-active manner whilst identifying opportunities for Home First. The worker must be able to follow policies and procedures, have excellent communication skills in addition to developed assessment and report writing skills. The worker will be required to provide daily updates on their caseload in relation to discharge planning.

Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments.

The worker must have the ability to travel throughout West Lothian.

Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.

This position requires registration with the SSSC. The successful candidate will have six months from the date of commencement of this post to become registered with the SSSC. SSSC stipulate you must submit your application within three months of your start date. If you are already registered with SSSC you will be required to add West Lothian Council to your account.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
