This is an interesting, exciting, and challenging opportunity for an enthusiastic individual to work within a ‘Locality’ working model. The role is mainly based in the West locality team but offers opportunities to work across West Lothian as required. The position also involves collaboration with the integrated first point of contact team within the central ‘Integrated Access Point’ (IAP).

This service initiative involves joint working across Health and Social Care partnerships. The team delivers support to individuals requiring assistance to maintain their safety and independence within the community. The role requires the ability to prioritize tasks, stay organized, follow procedures, communicate effectively, and produce good report writing. The successful candidate must demonstrate the ability to work as part of a health and social care team.

A recognized qualification in social care is advantageous, along with the ability to undertake essential training relevant to the role and in accordance with service standards.

The candidate should have a good understanding of Self-Directed Support and the significance of completing outcome-focused assessments. Training will be provided as part of the role.

The role requires a current driving license and the ability to travel throughout West Lothian.

This position is classified as Regulated Work with children and/or protected adults under the Protection of Vulnerable Groups (PVG) Scotland Act 2007. From 28 February 2011, preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check before a formal employment offer is made by West Lothian Council.
