The successful candidate will be based within the Integrated Access Point undertaking a range of social work services in relation to the assessment and care management of adults in West Lothian.

This will include ensuring service users meet eligibility criteria and considering community based, informal supports and technology to meet the clients needs.

The worker must have abilities to work as part of a team, respond to demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills.

The worker should be prepared to take the lead in more complex, high risk assessments as well as supporting other members of the team.

These tasks will be carried out in partnership with service users, family members, colleagues within the Integrated Access Point, social Policy and other professionals and agencies as necessary to meet need.

The worker will be required to undertake essential training including in adult support and protection training up to level 3 in accordance with service standards.

The worker must have the ability, if required, to travel throughout West Lothian and hold a recognised qualification in social care.

Relevant Social Work Qualification.

It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.

This post is considered Regulated Work with children and/or protected adults under the Protection of Vulnerable Groups (PVG) Scotland Act 2007.
