Assessment and Care Management Team – Autism and Learning Disability Team. The successful candidate will be based within the autism and learning disability team undertaking a range of social work services in relation to the assessment and care management of adults with autism and/or learning disability in West Lothian. The post holder will be assisting with care pathways, enabling people to remain as independent as possible. Assessment will take a strengths-based approach and post holders will be encouraged to think creatively about how we support service users during their period of assessment. This will include ensuring service users meet eligibility criteria.

The worker must have abilities to work as part of a team, respond to demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills. These tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need.

The worker will be required to have a good understanding of Self Directed Support and the importance of completing outcome focused assessments. The worker will be required to undertake essential training including in adult support and protection training up to level 2 in accordance with service standards. The worker must have the ability to travel throughout West Lothian and hold a recognised qualification in social care. Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.

The skills and knowledge candidate must have:

  • SVQ 3 and an HNC in Social Care or equivalent.
  • Relevant experience in a social or health care setting is essential.
  • It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
