We have an exciting new opportunity for a Band 4 Occupational Therapy Assistant within the Hounslow Community Neuro-rehabilitation Team and the unique Neuro Timely Supported Discharge Team to support our ability to meet the Integrated Community Stroke Service (ICSS) delivery model. This is an ideal post for an experienced Band 3 Rehabilitation Assistant / Occupational Therapy Assistant looking to further develop their neurology and management skills or a current Band 4 looking for a new challenge.
Responsibilities
- Work within a specialist neurological service with patients with a variety of neurological conditions.
- Take ownership of a small non‑complex caseload of your own whilst working in collaboration with a variety of clinical specialists within the team.
- Participate in in‑service training programmes and external training where appropriate.
- Participate in service and national audit activities including SSNAP and evidence‑based practice development activities.
- Contribute to rapid assessment, facilitating discharge from hospital, long‑term condition management and specialist neuro‑rehabilitation.
- Develop specialist skills with Parkinson's Disease and Multiple Sclerosis through MDT clinics.
Team
The Hounslow Community Neuro Rehab Team (CNRT) is a client‑centred, integrated multi‑disciplinary team comprising an Advanced Clinical Practitioner, physiotherapists, occupational therapists, a neuro psychologist, a pharmacist, specialist nurse practitioners, an occupational therapy assistant and rehab assistants. The neuro‑rehab team works across both intensive short‑term rehab and longer‑term rehab pathways.
