We are excited to offer a rewarding opportunity within the Care Coordination Service (CCS) for a Band 5 Health and Social Care Coordinator. You will play a key role in supporting vulnerable patients to navigate the complexities of health and social care, ensuring they receive timely, high-quality, integrated support.

Main duties of the job
- Working within a network of GP Practices to provide a central coordination role for patients identified throughout the health and social system and referred into the service.
- Responsible for a caseload of around 40/45 named patients with the aim of:
- Enabling people to remain at home,
- Reducing unnecessary admissions to hospitals,
- Supporting early discharge from hospital,
- Improving the quality of care,
- Encouraging independence.

- Relevant degree or equivalent level of training and experience.
