We are looking for a Registered Nurse to deliver high quality care to patients within Clayponds Hospital and Rehabilitation Unit in line with Trust policies, objectives, protocols and local guidelines, whilst also being aware of the importance of their role in supporting members of the team, and always adhering to the Trust values, and in accordance with NMC regulations.
As a centre of excellence, you will offer a person-centred approach to their independence and well-being. You will accept delegated responsibility for specific activities/areas from the senior nurse in charge. You will be able to work both autonomously and as part of a multidisciplinary team to deliver high standards of care to patients, communicate in line with Trust values, to patients, carers and staff at all times and take every opportunity to continue to develop, practice and expand their own general and specialist nursing skills.

About Clayponds Hospital: Clayponds Hospital is a 73 bedded, medically led rehab unit that provides personalised support and care to enable people to recover from periods of acute ill-health, regain function and prepare them to return home and back to their local communities. The unit has 3 different wards; Jasmine and Magnolia being the multi-pathology wards and Rosemary is a neuro ward. Each ward has 24 beds; 3 bays and 4 siderooms with a staffing level of: 1 Ward Manager, 3 RGNs and 3/4 HCAs and a Matron for the day. At night, there will be 3 RGNs and 2 HCAs for the ward and a Senior Nurse Cover for the whole unit.

Responsibilities:
As a Registered Nurse, you will be responsible for ensuring that patients are at the centre of the care you deliver, personal preferences are always put into consideration. As part of your role you will:
- Document care planning activities for patients to meet changing physical, psychological and social needs, taking into consideration their privacy, dignity, personal choice and consent.
- Ensure care plans are developed in collaboration with patients and their family/friends (where appropriate) and members of the multi-disciplinary team.
- Undertake all aspects of medicine management in line with legal, professional and regulatory requirements.
- Be responsible for promoting and safeguarding the welfare of patients and colleagues.
- Assess, plan, implement and evaluate individualized patient care through appropriate frameworks, including safe effective arrangements for discharge, underpinned by Trust values.
